ILASN is a network exclusively dedicated to serving and representing the needs of Illinois non-profit, post-acute, long-term care providers.

Members have access to resources, contract review and negotiating services, utilization reports and scoring that shows how their facilities rank both statewide and nationally.

Plus, members have a dedicated managed care team for contract negotiation, credentialing services, and a robust quality program.

To maximize the health and well-being of seniors in its member communities through innovative, cost-effective care management practices and quality improvement activities through contracting relationships with health plans and health delivery systems.
Meet The Team
Carolyn Roten

Partner Strategic Health Care
Carolyn’s more than 20 years of experience in health care include serving as the Director of Business Development for a large health care network where she presented and negotiated contracts with managed care organizations, working in the health insurance division of a national insurance carrier, and holding offices with the Ohio Continuum of Care Council, Continuity of Care Coordinators and serving on several health care provider boards. As a Partner of Strategic Health Care, Carolyn provides managed care contracting services to a broad spectrum of health care providers. She holds a degree with honors from Franklin University in Business Administration.
Arlene Jaroscak
Senior Vice President of Network Management and Quality
Arlene Jaroscak is the President of Innovative Long-term Care Advisors, a Licensed Nursing Home Administrator, Registered Nurse, Clinical Consultant, Conference Presenter, Speaker, and Author. Arlene received her Bachelor’s Degree in Long-term Care Administration from St. Joseph’s College of Maine and is a member of the Leadership Lorain County Class of 2009. As the Vice President of Network Management and Quality at Strategic Health Care, Arlene works with health care networks, managing their Quality and Contracting efforts, as well as the pay for performance initiatives of providers, including facilities, physicians and suppliers. Arlene’s visionary executive leadership in healthcare and her passion for quality and serving seniors, has traversed the long-term care continuum.
Jennifer Castore

Manager of Contracting and Client Compliance
Jennifer is the Manager of Contracting and Client Compliance and has twenty plus years of managed care and healthcare quality experience. Her current responsibilities include managed care contracting and network management related activities. Prior to joining Strategic Health Care, Jennifer gained experience working for a pediatric hospital system and its fully capitated Physician Hospital Organization, a Medicaid payor, and an adult hospital system and its Clinically Integrated Network. She has extensive knowledge of network development/management, contracting/negotiation, credentialing, managed care quality (HEDIS, Medicare Stars, PCMH), reinsurance, and process improvement activities. Jennifer is a graduate of The Ohio State University where she studied both Sociology and Public Health. She is also a Certified Professional in Healthcare Quality (CPHQ).
Grant Swemba

Director of Post-Acute Contracting & Alternative Payment Initiatives
As Director of Post-Acute Contracting and Alternative Payment Initiatives at Strategic Health Care, Grant brings over 20 years of comprehensive post-acute care knowledge across multiple service domains including skilled nursing, assisted living, home health, hospice, outpatient rehabilitation, and adult day care. His financial and operational contract development expertise is focused on building sustainability and ensuring that mandates are met. He offers essential knowledge vital to the development of effective alternative payment initiatives, including value based reimbursement and pay for performance models focused on quality outcomes and reduced spending
